My Care, My Choices is a Queensland Government advance care planning initiative.

Age, illness or accident may at some time make it hard for you to make decisions about your health care.

What is advance care planning?

Advance care planning involves thinking and making choices now to guide your future health care. It is also a process of communicating your wishes. If you have strong beliefs about what you want to happen in the future, it is particularly important to make your plans and wishes known now. You can do this by having a conversation with those close to you and writing down your preferences.

How to complete an advance care plan

It can be hard to talk about what might happen if you become very unwell, but it will give you and your loved ones peace of mind to know that your wishes can be known and respected. 

Queensland advance care planning forms

The Statement of Choices, Advance Health Directive, Enduring Power of Attorney and revocation of enduring document forms are Queensland advance care planning forms which can be used to record your wishes and choices for health care into the future. You can complete whatever form or forms you consider meet your needs.

Your patient may wish to write a less structured document but only the forms listed above can be uploaded to the person’s Queensland Health electronic hospital record.

About us

The Statewide Office of Advance Care Planning is a statewide service, hosted by Metro South Health, that helps to promote the important process of advance care planning. You can send a copy of completed advance care planning documents to the Statewide Office of Advance Care Planning for review and upload to your Queensland Health electronic hospital record (The Viewer). If you wish your documents to be in My Health Record only you can upload them via myhealthrecord.gov.au.
