Program ReACHing patients in their homes

Logan Hospital’s ReACH program has changed the lives of more than 155 patients over the last 12 months providing them with expert medical care in their homes and freeing up hospital beds for those acutely unwell.

The eight-bed Rehabilitation with Activity at Home service provides intensive rehabilitation to patients in their own home under the Hospital in the Home (HITH) inpatient model of care.

Unique to Queensland, Rehabilitation Staff Specialist Dr Deepa Iyer said patients on the ReACH program received a seven-day-a-week service with intensive multidisciplinary rehabilitation and management. 

“It provides patients with the opportunity to focus on person centred goals that are realistic, relevant, and meaningful within their own home,” she said.

“The ReACH service is in line with evidence-based practice which has clearly proven time and time again that most patients, specifically after cerebrovascular incidents or the elderly after any surgery, do better in their own environments.”

Dr Iyer said the ReACH service also freed up valuable inpatient rehabilitation beds for patients that required increased levels of care.

“The ReACH service was invaluable in moving patients from the hospital environment to home during the COVID lock down period, increasing capacity from eight to 12 to achieve this and that improved patient flow through the hospital.”

Dr Iyer said ReACH accepted patients with a range of diagnosis with achievable, functional goals for intensive rehabilitation at home.

“Over the last 12 months, 34 per cent of patients had a diagnosis of stroke or other cerebrovascular disorders, while 14 per cent of patients had a diagnosis of hip fractures.”

ReACH patient Lynette Reibelt said the program was wonderful and had helped her recover more quickly after suffering a stroke.

“It really felt like I have been getting better and each member of the team has played a special part,” she said.

“I felt more comfortable being at home and I was able to talk on the phone, do shopping – things I couldn’t have done in hospital and it really helped build my confidence.”
