Nurse Navigator team providing complex care

Our Logan Hospital Nurse Navigators are continuing to provide advanced clinical and professional care for patients despite a challenging time over the last several years.

Nursing Director Kathy Flanigan said the team worked hard to facilitate safe and efficient patient journeys.

"Our Navigator team cares for patients across a range of services including diabetes, dementia, bariatrics, disability, heart failure, geriatrics, care of the elderly, rheumatic heart disease, renal, movement disorders as well as providing a service for people from multicultural backgrounds and frequent presenters to the emergency department," she said.

"Their roles are important one;  improving complex patient care, increasing system efficiencies, reducing costs by streamlining and coordinating care while forming partnerships and ultimately improving patient outcomes." 

Kathy said when asked recently how they would describe their roles one of her favoutire responses was "Navigators work as a client advocate, a detective, a cheerleader, and a health coach who provide flexible, client led services that focus on securing services for clients whilst enhancing client’s health and wellbeing".
