The Nurse Navigator Generalist service covers the entire Metro South Health (MSH) service district, providing a key point of contact for the patient, reducing fragmented care, providing an end to end service. The Nurse Navigators will work closely with the Primary Care team, facilitating the care pathway for patients having difficulty navigating the health system and managing self-care.
