CAREPACT team win Ministers award for excellence

The Metro South Health CAREPACT team has been awarded the Minister’s Award for Excellence in the Annual Queensland Health and Department of Health Awards for Excellence held last week.

Comprehensive Aged Residents Emergency Partners in Assessment Care and Treatment (CAREPACT) is a unique demand management program that focuses on improving the care of residents in Aged Care at every stage of the care continuum. The coordination of care honours the individual rights and personal choices by providing the right care, at the right time, in the right place.

CAREPACT's vision is to empower and enable aged care residents to live the final years of their lives with dignity and in accordance with their individual values and care goals, through the timely advocation of their voice and care needs, in the right location.

The Minister’s Award for Excellence acknowledges a person/team who has demonstrated a commitment to excellence and achieving outstanding results.

The CAREPACT team have certainly demonstrated this, and the expansion of the CAREPACT model from across Metro South Health to other Hospital and Health Services as the Residential Aged Care Facility (RACF) acute care Support Services (RaSS) model is a testimony to best practice.
