My Care My Choices: Metro South Health End of Life Programme

Project Status: 

My Care My Choices programme recognises that Advance Care Planning (ACP) needs to be embedded into routine care across all medical specialities and environments of care. Success of the programme is dependent upon clinical cultural change and public acceptance. The programme has received recognition state wide with various Health and Hospital Services requesting use of the developed Statement of Choices ACP documentation, access to reporting systems to meet funding requirements and storage of ACPs for easy retrieval by Queensland Health clinicians.  

My Care My Choice objectives for 2015-16 include:

  • Provide governance and support through the continuation of the MSH End of Life Steering Committee and Working Party. 
  • Increase public awareness of ACP benefits across MSH through a multimedia communication strategy.
  • Development of online standardised clinical education initiatives providing a framework of care for end of life care that is adaptable across all settings of care including MSH public hospitals 
  • Trials to illustrate benefits of the use of ACPs in teaching hospitals. 
  • Development of efficient systems to upload ACPs onto the Viewer.
  • Development of policies and procedures that support the adoption of common language, storage and retrieval of ACPs.
  • Establishment of ACP clinics in community settings.
  • Development of quality, audit and evaluation processes.
Aims & Goals: 

This project aims to implement a sustainable and integrated end of life programme, My Care My Choices, across Metro South Health (MSH).
