Nurses work alongside Micah’s existing Street to Home workers to form a care team that visit residences of vulnerably housed people and public spaces/locations every night to deliver a range of medical and non-medical services, both planned and opportunistic, to the homeless and vulnerably housed. The nurses link with a care coordination day hours nurse to effectively link these people into day hours services including healthcare and social assistance providers and to the acquisition of public housing.
• Improved access to the right care, at the right time, in the right place for the homeless
• Improved timeliness of care/interventions
• Service profile meets the needs of the population
• Reduced potentially preventable hospital admissions, emergency department presentations and/or length of stay
