The Integrated health strategy 2015 - 2017 is a living document that has been developed by the Integration team who are part of Metro South Hospital and Health Services (MSH) Planning, Engagement, and Reform Directorate and builds on current and proposed integration activities across MSH. The strategy has been developed in consideration of existing MSH stream, team, and hospital avoidance plans. Planning and implemention of the strategy will be via project methodology. The strategy includes the vision, goal, roles stategic priorities, how success will be measured as well as an overview of the governance and business model concepts.
The strategy has been developed to align with MSH strategic plan 2015 -2019 focus area 3 : improved health system integration.
Our Objective: Working together for a better health system through 1) Partnerships: Partnering across sectors to ensure patients have access to high quality care when and where they need it. 2) Information and communication: Improving the flow of information across sectors to enable better communication between service providers and a seamless patient journey 3)Access to health services: Improving access to health services for all population groups. 4)Population wellness and prevention of disease: Supporting cross sector health promotion strategies to enable the community to better manage their health
How we will measure success Burden of disease as measured by disability adjusted life years Reduced potentially preventable hospitalisation Increased proportion of people with Chronic conditions who have received an annual cycle of care with in general practice (per 100 population) Improved patient experience Increased rates of screening for target populations, breast, cervical and bowel cancer Increased rates of immunisations for vaccines in the nation schedule
