Cognitive Impairment Rounding Model

Project Status: 

This project supports staff to consistently consider the following when providing patient centered care:

  • Identify cognitive impairment, delirium and risk factors
  • Prevent and treat causes of delirium
  • Provide care based on the individual's needs to improve the outcome of their care

The focus of the rounding model is on the implementation of evidence-based strategies to prevent or treat delirium. It utilises allocated nurses to focus specifically on delirium prevention and management strategies, as separate from primary nursing duties.

Aims & Goals: 

The aim of this trial is to conduct a successful change to the existing model of care, transitioning from an individual specials model to the cognitive impairment rounding model.
