CARE-PACT Expansion Project

CARE-PACT is a collaboration between Metro South Health and Hospital Service, Wound Innovations and Brisbane South PHN to improve the management of elderly patients with chronic wounds in residential aged care facilities who would otherwise require emergency department or hospital management. This project will utilise new pathways, technology, and workforce strategies.

Facing up to Chronic Disease - Diabetes

Following extensive stakeholder and community engagement, the Metro South Health (MSH) Board and Brisbane South PHN Board have endorsed the implementation of the Facing up to Chronic Disease program. The strategy is a long-term plan to connect care and improve the health and wellness of the MSH population who are living with chronic disease, and reduce the social, human and economic impact of this.

The GDM App and Clinician Portal, also known as M♡THer, is a mobile health system to support monitoring and management of Blood Glucose Levels for women with gestational diabetes mellitus.  M♡THer is a collaboration between Redland Hospital, the Transformation Innovation Collaborative, and CSIRO.

MSH Value Based Chronic Wound Care

The MSH Value-Based Wound Care Project was developed to respond to an increased number of patients presenting to hospital with wounds and associated avoidable admissions, the lack of a co-ordinated service and referral pathways, long waits to access specialty outpatients, community disengagement from the service and lack of co-ordinated post-acute care follow up across Metro South Health.

PD Buddy App and Portal

The Peritoneal Dialysis (PD) Unit at Logan Hospital has been working with CSIRO and the MSH Transformation and Innovation Collaborative (TIC), to develop an application (App) and web portal that enables patients requiring PD to record, store and review treatment information on their mobile devices.The web-portal enables clinicians to view their client's progress and provide individualised care. This project is currently in the pilot stage.
