Better Aged Care

This project aims to prevent unnecessary hospital transfers, and improve patient care outcomes by providing a service that will directly decrease the number of non-acute hospital services/unnecessary hospital transfers by the Queensland Ambulance Service (QAS) from Residential Aged Care Facilities (RACFs) in the after-hours period.

Cardiac Rehab- using technology for community rehabilitation

The Cardiac Rehabilitation project will increase access to cardiac rehabilitation (CR) services and programs across Metro South Health. 

CARE-PACT Expansion Project

CARE-PACT is a collaboration between Metro South Health and Hospital Service, Wound Innovations and Brisbane South PHN to improve the management of elderly patients with chronic wounds in residential aged care facilities who would otherwise require emergency department or hospital management. This project will utilise new pathways, technology, and workforce strategies.

Chronic Disease Diabetes Model of Care Redesign

This project aimed to deliver an efficient acute model of care to assist in service delivery and integration of diabetes services across Metro South Health.

This project will trial a new model of care for patients with cognitive impairment, which utilises evidence based strategies in determing appropriate, and individualised, provision of care.

Facing up to Chronic Disease - Diabetes

Following extensive stakeholder and community engagement, the Metro South Health (MSH) Board and Brisbane South PHN Board have endorsed the implementation of the Facing up to Chronic Disease program. The strategy is a long-term plan to connect care and improve the health and wellness of the MSH population who are living with chronic disease, and reduce the social, human and economic impact of this.

Feed not fast: leading the way in hospital patient nutrition

Staff at QEII Hospital have been working hard to improve the nutrition of patients with their Feed not Fast project.

Gastroenterology/Hepatology Integrated Clinics

This project aims to establish multidisciplinary Gastroenterology and Hepatology Integrated Care Clinics that provide a platform to identify clinically suitable category 3 gastroenterology and hepatology patients for management within the community. This project is supported by the Integrated Care Innovation Fund (ICIF).

Group Maintenance for Communication in Parkinson's Disease

The speech pathology services of QEII Jubilee and Community Adult Rehabilitation Services have piloted an innovative speech and language treatment for people with Parkinson's Disease. The Loud and Proud program has been reviewed, and an investigation of student-led intervention is underway.
