Gastroenterology/Hepatology Integrated Clinics

This project aims to establish multidisciplinary Gastroenterology and Hepatology Integrated Care Clinics that provide a platform to identify clinically suitable category 3 gastroenterology and hepatology patients for management within the community. This project is supported by the Integrated Care Innovation Fund (ICIF).

Integrated Respiratory Access Project

The Integrated Respiratory Service provides holistic, patient-centred care in the form of education, information and support to patients and their families and carers within the Logan catchment for the diagnosis of chronic obstructive pulmonary disease (COPD), asthma, bronchiectasis and pulmonary fibrosis, prescribed home oxygen and smoking cessation.

The Integrated Respiratory Access Initiative is a new service funded by the Integrated Care Innovation Fund (ICIF) that will expand the existing  model by increasing the number of clinics that are provided and by transforming the way that current services are provided. The initative aims to reduce avoidable Emergency Department presentations and alleviate General Practitioner and patient concern.

QEII Older Persons Coordinated Care Service Project

This project aims to improve access to coordinated care for complex, frail, older people.  The key components of the initiative are a single service with a fully integrated multidisciplinary team; improved care including carer and staff education; the ability to explore care options in the community; and enhanced communication strategies with a focus on timely coordinated discharge communication.

This project is funded by the Integrated Care Innovation Fund (ICIF) and will be undertaken in partnership with Queensland Health's Healthcare Improvement Unit and the Brisbane South PHN.

Zika Mozzie Seeker

The world-first ‘Zika Mozzie Seeker’ project aims to increase our confidence that mosquitoes with the potential to spread Zika or dengue viruses (Aedes aegypti species) are not present in the Metro South Health region.
