This project aims to prevent unnecessary hospital transfers, and improve patient care outcomes by providing a service that will directly decrease the number of non-acute hospital services/unnecessary hospital transfers by the Queensland Ambulance Service (QAS) from Residential Aged Care Facilities (RACFs) in the after-hours period.

Facing up to Chronic Disease - Diabetes

Following extensive stakeholder and community engagement, the Metro South Health (MSH) Board and Brisbane South PHN Board have endorsed the implementation of the Facing up to Chronic Disease program. The strategy is a long-term plan to connect care and improve the health and wellness of the MSH population who are living with chronic disease, and reduce the social, human and economic impact of this.

Gastroenterology/Hepatology Integrated Clinics

This project aims to establish multidisciplinary Gastroenterology and Hepatology Integrated Care Clinics that provide a platform to identify clinically suitable category 3 gastroenterology and hepatology patients for management within the community. This project is supported by the Integrated Care Innovation Fund (ICIF).

Julian's Key health passport app

This project will trial the use of a health passport app which documents the care and support needs of people with disabilities while they use our health services at Logan Hospital.

This project aims to increase health equity and access of Metro South Health Culturally and Linguistucally Diverse (CALD) consumers by developing resources that are culturally and linguistically accessible and appropriate, utilising innovative technologies, applications and/or software to improve accessibility, and developing internal processes to promote best practice resource development.

Michah Homeless to Home Health

Homeless to Home Health provides nursing care to the homeless population within inner city Brisbane and Brisbane City Council wide boundaries.

QEII Older Persons Coordinated Care Service Project

This project aims to improve access to coordinated care for complex, frail, older people.  The key components of the initiative are a single service with a fully integrated multidisciplinary team; improved care including carer and staff education; the ability to explore care options in the community; and enhanced communication strategies with a focus on timely coordinated discharge communication.

This project is funded by the Integrated Care Innovation Fund (ICIF) and will be undertaken in partnership with Queensland Health's Healthcare Improvement Unit and the Brisbane South PHN.
