Providing your details

When visiting our hospitals and facilities, you may be asked to provide your contact details in the event we need to reach you as part of our COVID-19 contact tracing efforts.

Visitor restrictions at our facilities due to COVID-19: Please refer to our hospital visitor restrictions for current information on visitation restrictions at our facilities and mask requirements. If you have any symptoms or are concerned about your exposure to COVID-19, you can visit one of the fever clinic locations in Metro South. Thank you for your support and understanding during this time.

At some hospitals, you will be asked to scan a QR code using your smart phone and enter your:

  • First and last names
  • An email OR residential address
  • Phone number.

You may also be asked what areas of the hospital you are visiting. If you do not have a smartphone to scan the code, our staff will be able to assist you to check-in if required.

Data collected via the QR code is safely stored in accordance with Metro South Health's privacy policy and will only be accessed if contact tracing is required. It will be deleted after 56 days.
