Developed by Hepatitis Queensland, the Kombi Clinic will now support more people living with Hepatitis C. The Kombi Clinic Project is a collaborative mobile clinic operated in partnership with Dr Matt Young and Dr Joss O’Loan from Medeco Medical Centre, Inala. It targets hard to reach or otherwise disadvantaged populations where there are known high rates of incidence, prevalence and/or risk of transmission of Hepatitis C.
Anglicare Southern Queensland will begin an innovative pilot project providing community-based wound care and education. The Wound Clinic Pilot Project will provide evidence-based care in the community and enhance the knowledge of care providers, clients and their families to better manage and prevent this very complex health problem.
The World Wellness Group aims to reduce the number of preventable cardiac hospital presentations by people from culturally and linguistically diverse (CALD) backgrounds. The language barrier, and limited knowledge of the Australian healthcare system, means people from CALD communities are being discharged with treatment plans and medication regimes they don’t fully understand. The project will take a multifaceted approach including better community coordination and developing collaborative care protocols to help overcome these difficulties.
