Living Healthier Lives Community Grants

Living Healthier Lives is a small grant program aimed at supporting local community initiatives to make a positive difference to the health and wellbeing of people in the Metro South region.

Grants of between $20,000 and $100,000 are available to support existing initiatives or to develop new ones.

What is the focus of the grants program?

The key focus of the 2018 program is "helping people to manage their health conditions in the community and keeping well".

We are particularly keen to hear from organisations who have projects that will help reduce potentially preventable public hospital admissions for Metro South Health facilities. The ten most common potentially preventable admissions at Metro South Hospital facilities include:

  • Diabetes complications
  • Chronic obstructive pulmonary disease (COPD)
  • Congestive heart failure
  • Cellulitis
  • Urinary Tract Infection
  • Angina
  • Influenza and Pneumonia
  • Ear, Nose and Throat Infections
  • Convulsions and Epilepsy
  • Gangrene

Who is eligible to apply?

To be eligible you must:

  1. Be located in the Metro South region
    • Metro South region covers the Brisbane southside, Logan, Redlands and Scenic Rim (to Beaudesert) regions
  2. Be a Community Applicant (one of the following):
    • An incorporated not-for-profit organisation
    • Organisations sponsoring an unincorporated not-for-profit organisation or
    • An individual officially representing either of the eligible organisations above.
