The Advance Care Planning (ACP) Week team spruiked an important message across Logan and Beaudesert Hospitals last week as part of National ACP Week.

From digital screens to newsletters, screensavers, and foyer displays, the 'Life’s better with a plan message' was front and centre.

The Metro South Health ACP Service team engaged with patients, families, and clinicians to highlight the importance of planning for future healthcare needs.

Among those stopping by the vibrant ACP display at Logan Hospital were Kath Zabala, CN CHIP Logan (left), and Jeminah Paisa, Registered Nurse, Nursing and Midwifery Resource Unit (right), who took the opportunity to chat with ACP Service Clinical Nurse Consultants.

"I believe advance care planning protects patients’ rights and ensures their choices are honoured if they lose capacity," Kath shared.

"As a nurse, it's important for me to understand how my patients want to be cared for and to respect their wishes if they can’t speak for themselves," added Jeminah.

The Metro South Health ACP Service provides specialist support to help patients, families, and communities discuss, plan, and document their healthcare preferences. Free monthly information sessions are available at Community Health Centres across the region.

Clinical Nurse Consultants also support clinicians across all MSH facilities, providing direct patient and family assistance as well as education.
