Happy first birthday to our GEMITH team

It has only been 12 months but already Logan Hospital's incredible GEMITH team has revolutionised treatment for geriatric care.

As they celebrated their first birthday recently, social worker Grace Baldwin shed light on the program’s impact and outlined plans for its future.

Starting as a group of individuals with a shared passion for aged care, Grace said the GEMITH team had developed into a robust, collaborative unit that continued to strive for excellence and best outcomes for the GEMITH clients and their families.

“I believe it is the unwavering teamwork that has driven the success of the program,” she said.

Grace said patient feedback continued to highlight the positive impact of the program and fuelled the team’s passion to further improve the GEMITH program in the future.

She said while the past twelve months hadn’t been without its challenges, the team was happy to report that all 218 patients through the program had reported greater confidence and resilience and a decreased risk of relapse or unnecessary hospital readmission.

"The GEMITH team’s year ahead will focus on a commitment to continuous improvement including research initiatives and the integration of professional tools to assist with data collection and outcomes."
