Our commitment statement

Our commitment is to give you access to the information held within our health service, unless it's in the public's best interest for us to keep it private.

What is Right to Information

The Right to Information (RTI) process is the Queensland Government's approach to giving the community greater access to information.

Two pieces of legislation define this process:

  1. The Right to Information Act 2009 (RTI Act)—allows you to apply for access to documents held by Queensland Government agencies.
  2. The Information Privacy Act 2009 (IP Act)—details privacy principles and allows you to apply for access and to make amendments to personal information.

The RTI and IP Acts help ensure that:

  • our community has access to our information
  • we protect people’s privacy.

You can learn more about RTI requests on the RTI website.

Making a RTI request

Before making a formal request under the IP Act, ask if your personal information is available through our administrative access process.

Find out more about our administrative process and applying for your health records.

Check what information is already available

Before you apply, see if the information is available on one of these Queensland Health websites:

If you can’t find the information you need before making a request, contact the Information Access team. They’ll help you with no need to make a formal request, by:

  • directing you to information that is already available
  • supplying copies of information that is already public
  • helping you update your personal information.

How to make a formal RTI request

You have the option to:

Submit the PDF form either in person, by post or email to the specific Information Access Units that your request is about.

Fees and charges

You'll need to pay an application fee and there may be other costs. We’ll let you know how much your application will cost when we get it. Learn more about fees and charges on the Right to Information website.

After you make a request

We'll look at your request and write to you about our decision within 25 business days. We'll also let you know if we need more time to process it. This can happen if we need to involve another government agency or organisation.

We’ll notify you if the documents you're requesting don’t exist or if we don’t have them. We might also remove information from the documents if it’s not relevant to your request. If we can't provide you with the information requested or if we can't release it to you, we'll explain why.

Review rights

If you’re not satisfied with our decision, you can ask us to do an internal review. You can also apply for an external review with the Office of the Information Commissioner.

You must apply for a review within 20 business days from the date of our decision.

Disclosure log

We publish all non-personal RTI requests in our disclosure logs, which include descriptions of the documents released.
